Acknowledgement:  The STUDENT, by applying personally or by telephone, acknowledges reading and understanding the information contained on this Application Form.  The TRAINER reserves the sole and exclusive right not to accept STUDENT’s Application form.

I understand that the course of instruction will prepare me to actively invest / trade securities and/or other financial instruments only for my own account at an appropriate financial firm which utilizes electronic transmissions to execute stock exchange orders for its customers.  I understand that this course is not preparation to be a Licensed Broker in the financial industry.

Representations. The STUDENT acknowledges that the TRAINER has no representations or guarantees other than those expressed herein; and depends upon the individual skills, time availability and dedication of each applicant to the Training Program.  This Agreement represents the entire understanding and supersedes all prior statements, agreements and understandings, if any, either oral or written.  Moreover, the Training Program should not be construed as a recommendation nor an offer to buy or sell any security or the suitability of investment strategy for STUDENT.  The purchase, sale, or advice regarding any security, other financial instrument or system can only be performed by a licensed Industry representative; such as, but not limited to a Broker/Dealer, FCM and/or Registered Investment Advisor. Neither the TRAINER nor its representatives are licensed to make such advisements. PFTI trainees are encouraged to speak with a licensed representative of their choice regarding the appropriateness of investing/trading or of any particular investment/trading strategy. 

Guarantee Policy. The TRAINER grants each STUDENT the privilege to repeat any class identical to that which the STUDENT has previously attended during normally scheduled class hours on a space available basis, and at those times during which that particular class may be available, provided that the class being repeated by the STUDENT was originally paid for in full in a timely manner.  The TRAINER reserves the right to define “space availability” .

Rescheduling. A fully paid STUDENT who is unable to take a class on the designated date due to illness or unforeseen circumstances may reschedule to take the class when it is again available without penalty, provided written notice is provided to the TRAINER at the earliest opportunity available to STUDENT and no later than ten days of the initially scheduled class.

Refund policy.  No refunds are given 21 days prior to the start of the Original Class Date.  If within three calendar days from the date of STUDENT’s first payment towards taking a class, the STUDENT notifies the TRAINER in writing of the desire to withdraw, AND the notice itself is received by the TRAINER more than twenty-one (21) days prior to the initially scheduled start date for the STUDENT’s course (original class date), the TRAINER will issue a full refund within 30 days, from the date of notice, in the amount actually paid to the TRAINER by STUDENT, provided all training materials have been returned in undamaged and unopened condition.  If training materials have been opened or are otherwise damaged or not returned, the cost of the training material and applicable sales tax will be deducted from the refund.

I understand that no videotaping or voice recording is permitted during class without written approval from PFTI management.  I understand that the TRAINER may terminate my participation in the course at any time in its sole discretion.  If the TRAINER makes such determination, it will promptly refund the proportional tuition that correlate with the untaken portion of the class. All online courses are non refundable.

I hereby permit the TRAINER to release my contact information to broker dealers affiliated with the TRAINER in order to receive tuition reimbursements as well as their promotional material.

The parties acknowledge that they have required that this agreement and all related documents be prepared in English.  Les parties reconnaissent avoir exigé que la présente entente et tous les documents connexes soient rédigés en anglais.
